they sell worm hooks where ever you can buy plastic worms.  Look for sluggo's (great pike plastic btw) and you should find the hooks.

They are pretty typical other then the first 1/4 by the eye has a dog leg bent into it.  

To be real honest you can use any long shanked hook with pretty good success and save yourself a few bucks.  That or bend them yourself.

You should be able to find pictures of them in something like a bass pro or cabelas catalog.
