They were made by a gunsmith from a solid piece of steel. They are drilled and tapped and have the same overall outward shape of the factory thumb nut. But they are smooth and contoured having a small shaped lever instead of the screwdriver head.

You index the lever by using thin washers under the nut. Mine work well.
